In summation, companion viruses will search for an EXE file and create an identically named file with a .COM file extension. Since you can execute files in DOS without specifying an extension, DOS will always run the file with a .COM extension first, which will run the virus. The virus then in turn runs the regular EXE file to prevent suspicion on the part of the user.
